As a specialty practice, we do not accept direct payments (also known as “assignment”) from insurance companies. As such, patient’s are responsible for fees charged. We will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
If you have dental insurance, we are be able to submit an electronic claims to your insurance company directly. In order to do this we will require the following:

The Insured’s full address

Name of Insurance Company

Policy# and Certificate #
