First Visit

Your initial appointment will consist of a consultation during which the Doctor can explain your diagnosis and available treatment options. We understand our patient’s busy schedules. As such, we make every effort to carry out your endodontic treatment the same day as your consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Prior to your first visit, we encourage patients to fill out their patient information online. This data is encrypted and transmitted to our secure server. This will save you time in our office filling out paperwork. Online registration can be done on this website by selecting the Patient Registration tab under the Patient Information menu. Alternatively, a paper version can be downloaded at filled out at home and brought to your first visit.
Please assist us by providing the following information at the time of your reserved appointment:
  • Your referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring your policy information. This will save you time and allow us to help you process any claims.

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they will be taken by us.

IMPORTANT: A parent or guardian must accompany all patients under 18 to their appointment.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require antibiotics prior to dental cleanings.
